Following CDC approval of the covid vaccine for children ages 12-15 we anticipate many of our students are getting vaccinated. The health office would like to collect this information. If your child receives the covid vaccine please forward a copy to our office. Please note, we need proof that your student received both doses, so only send along documentation once they receive their second vaccine. Thank you so much!
