The Massachusetts Department of Public Health requires that all students in grade 7 (as well as 1, 4, and 10) have a current copy of their updated immunizations and physical exam on file in the school’s Health Office.
We are asking that you please send in a copy of your child’s most recent physical exam with a record of their updated immunizations. You can also email a copy of these items to ktobyne@beverlyschools.orgIf your child has a physical exam scheduled in the near future, please send in a copy to the Health office as soon as their exam is completed. You can also have the doctor’s office fax a copy to the school’s health office at fax# 978-529-2170.
