Students with Asthma & Severe Allergies

This is a friendly reminder from the Beverly Middle School Health Office.

If your child has severe asthma or life-threatening allergies, please send in an extra inhaler or EpiPen to be kept in the clinic at school. All medications kept in the health office require a signed parental/guardian medication form and a signed physician medication order by a licensed prescriber.

Students who need to carry their own inhalers or EpiPens must have written self-administration permission from the licensed prescriber and the parent/guardian.

Attached is the Medication Administration Parent Information, a copy of Beverly Public Schools Physician Medication Order and Parental Consent.

If you have any questions or concerns please reach out to us at (978) 921-6103 Option 7, or email jelder@beverlyschools.org, emoffett@beverlyschools.org or ktobyne@beverlyschools.org.

Thank you!
